RETURNING CUSTOMERS WHO HAVE PREVIOUSLY PROVIDED US WITH THEIR PATIENT ACKNOWLEDGMENT FORM, IDENTIFICATION AND MEDICAL QUESTIONNAIRE do NOT need to provide these to us ever again as we keep these on file. However, please provide us with an updated Medical Questionnaire if there have been changes to your health status since the last time you provided us with this document.
RETURNING CUSTOMERS WHO ARE ORDERING PRESCRIPTION ITEMS WHICH HAVE REFILLS REMAINING ON THE PRESCRIPTIONS PREVIOUSLY SENT TO US are only required to fax or mail us the completed Order Form. You do NOT need to provide us with your prescriptions as we already have these on file.

- the completed Medical Questionnaire
- doctor's prescription
- a copy of two pieces of identification (one ID must be a government issued photo ID and the other can be any form of ID with your full name on it such as a birth certificate, voter registration card, utility or phone bill, etc.)
